Patient Centered Medical Homes

While primary care physicians are best positioned to take on coordination of care functions, many cannot assume this role effectively due to time, resources, and reimbursement constraints. Many organizations have recently embarked on Patient-Centered Medical Home (PCMH) demonstration projects, and several are experimenting with the use of a dedicated care coordinator to help eliminate gaps in shared information and communication among providers and between providers and patients.

The purpose of this 1-year research study is to develop and test the feasibility of using a social media platform for capturing and exchanging care coordinator’s sharable tacit and explicit knowledge across different organizations. The goal is to define best practices for creating patient centered clinics. Provided support to the research team for:

    Building a framework for capturing and mobilizing tacit and explicit knowledge across organizations Utilizing social media platform for knowledge sharing between care coordinators Initiating design, development, and implementation of a collaborative environment
